Because OCD can manifest in so many varied ways that its diagnosis is often overlooked or misunderstood as there are so many different symptoms of OCD, Pure O, Harm OCD, Relationship OCD, hypochondria, and all the various compulsions and intrusive thoughts.
OCD sits on top of anxiety and other anxiety disorders.
John Glanvill gives a detailed explanation of what OCD is from a former OCD sufferers perspective. Obsessive compulsive disorder is more common than most people expect. It ranges from intrusive thoughts through panic attacks and for some, a range of compulsions that force the experiencer to repeat certain routines that may include, touching, counting, checking, cleaning.
At its core are unrequested, intrusive, and obsessive thoughts that cause you to create scary stories in your mind, of all the things that could go wrong, thus invoking anxiety in your body, doubt about the right thing to do, and the need for reassurance.

Pure O has many variants including Harm OCD / sexual / religious / suicide - and other Obsessive and intrusive thoughts.
In this short video, I (John Glanvill) would like to talk about a form of OCD referred to as Pure O. Where a person has scary and intrusive thoughts - but seemingly lack the compulsions - like; checking, counting, touching, cleaning and things like that - which are more commonly associated with conventional Obsessive-Compulsive Disorder.
These unrequested and intrusive thoughts seem to follow similar patterns which include; thoughts of harming or killing others which often leads to fearing being around knives or other weapons. Thoughts of a sexual nature about others or fears you may sexually abuse a person. Intrusive thoughts about God, blasphemy, sinning or being judged by your deity.
For some, the thoughts may become fixated on suicide, becoming schizophrenic - or that you may be gay or transgender.
Pure O is something I have experienced myself - and in my capacity as a therapist, I've helped many people bring it under control - by helping them to understand what it is and how to manage their attention and awareness.
We can manage (or overcome) Pure O by learning more about what thoughts are, how to ignore them, then how to retrain our brains (and calm our bodies) via ERP (exposure response prevention).
Then exploring the fact that you probably are a little more extroverted than most people with conventional OCD though the rebellious or dominant side of your personality is probably being suppressed.
One pattern not often mentioned is that many Pure OCD sufferers do (just naturally) have very creative minds which they seem to be using against themselves rather than for their own good. I would guess that if Stephen King had some scary thoughts he would enjoy them and turn them into a book?

Can we supplement science and medicine by coupling the mind of an engineer who brings logic and reason to the world of stress and OCD, which are based on chemically driven emotions and automated neurological and biological functions?
You can't think your way out of anxiety or its darker cousin OCD (Obsessive Compulsive Disorder) - because these are conditions of thinking and worrying too much!
The skill of thinking was one of the last developmental phases of the human species; therefore (evolutionarily speaking), we are emotional animals who learned to think, not thinking creatures who try to control emotions.
Until the science-based mainstream medical world can see this and approach anxiety disorders in new ways, we will always be treating the symptoms of anxiety rather than going to the source - the unconscious mind and the limbic brain, where profound behavioural changes may be re-registered relatively easily.
Then, unless the individual can discern who they are, decide what they want and develop the courage and skills to step out and try to get it - they will likely be living a life, wherein the closing stages, they may have many regrets.
