So I’ve got an email from a reader who says, “My mom is in ICU with multi-organ failure with underlying severe acute pancreatitis. She’s ventilated requiring 50% FiO2 support on the ventilator, blood pressure support and dialysis. What are our next steps? What questions should we ask regarding further treatment? Today, she has had a cytosis drained via ultrasound, endoscopy, and fluid drain from a pulmonary edema. 700 mls was drained. She’s only 67 years old”. Thank you so much for this question. Let’s break down what’s happening. Unfortunately, many patients in ICU that end up with pancreatitis can go into a multi-organ failure, but I’ll come back to that in a minute. So currently we are working with a client who has their 58-year old husband/father in intensive care after a cardiac arrest and a hypoxic brain injury. The client has been in ICU now for about 10 days. The ICU team as usual was very adamant from day one to stop life support because they were saying that the client will never wake up and will never have any quality of life so that they wouldn’t want to live anyway. Now the family kept pushing back from day one saying, “Look, we want everything to be done. We don’t agree to withdrawal of life support.” And that they want their loved one to live irrespective of the quality of life they may or may not have in the future. Now, one of our readers wrote to us that, one of their biggest frustrations is that their loved one has brain damage after major heart surgery. Now, when does that happen? And how does it happen? So, what I’ve seen over the years after having worked in intensive care for over 20 years in three different countries, where I have also worked as a nurse unit manager for over 5 years, I’ve seen that sometimes after open heart surgery, patients end up with a stroke. And that is very unfortunate and that’s one of the ways brain damage can occur. So yesterday, I had a client call me saying that her four-year-old daughter has been in ICU for the last two weeks. She was in a near drowning event about two weeks ago. She probably sustained some form of brain damage, and she hasn’t woken up properly. The client couldn’t give me a Glasgow Coma Scale as she simply didn’t understand what a Glasgow Coma Scale is and how it works, and all of that. Which then brings me back to that, the biggest challenge for families in intensive care is always that they don’t know what they don’t know. They don’t know what to look for. They don’t know what questions to ask. They don’t know their rights and they don’t know how to manage doctors and nurses in intensive care.
