So yesterday, I had a client call me saying that her four-year-old daughter has been in ICU for the last two weeks. She was in a near drowning event about two weeks ago. She probably sustained some form of brain damage, and she hasn’t woken up properly. The client couldn’t give me a Glasgow Coma Scale as she simply didn’t understand what a Glasgow Coma Scale is and how it works, and all of that. Which then brings me back to that, the biggest challenge for families in intensive care is always that they don’t know what they don’t know. They don’t know what to look for. They don’t know what questions to ask. They don’t know their rights and they don’t know how to manage doctors and nurses in intensive care. And this is exactly what this young mother is dealing with here, that she doesn’t even know what she doesn’t know... Now, today I want to comment about a case of the NDIS (National Disability Insurance Scheme) where there were recent funding cuts and I just want to play a media report. While some of it might be really distressing, but it just illustrates what’s going on at the moment, and I think people need to know about it and they also need to know that we are a solution for this here at Intensive Care at Home. Let’s just play the media report of a little girl who had their nursing funding cut from the NDIS. So, let’s just play this from Ben Fordham from 2GB Radio: “The crunch is coming for the National Disability Insurance Scheme. After letting the cost of the NDIS explode, they’re now trying to contain the extent of the damage. Families are receiving phone calls and they’re having their funding cut, and here’s why. In recent years we’ve allowed people to charge all sorts of weird and wacky stuff onto the NDIS, from brothel visits to luxury cruises. And now we can’t afford the vital care that people really need. Alicia writes, ‘The NDIS couldn’t even afford my family, the decency to speak to me. Instead, they emailed my support coordinator at 4:59 p.m. Her three-year-old daughter, Koa, has an incurable brain disorder and the NDIS has just decided to swing the ax on her support. Now, what I’m about to play you is distressing, but this is how Koa’s sister, Ava, reacted when she heard what was happening to Koa. ‘She doesn’t live without all this help,’ she said. ‘Will Koa even survive without all this help?’ That’s her sister. According to Koa’s mum, the NDIS has now taken away nursing supports, a wheelchair accessible vehicle, music therapy, and hydrotherapy. Alicia writes, ‘The biggest cut is to nursing support. They’ve done this to my little girl who experienced more than 120 seizures in one month and who relies on oxygen for life support to stay alive. So we’re often hearing families asking us, what’s the difference between hospice and palliative care when patients are in intensive care? Now, as you may have heard me say before, especially if you are a regular viewer of my videos or my blog, that ICUs are very quick to want to withdraw life support, especially at the time of this recording with COVID-19 going rampant all across globally, really, and ICUs are full. ICUs are struggling for bed capacity. So we are certainly seeing that more and more ICUs are trying to push, especially COVID-19 patients, towards hospice fairly quickly. Hospice basically is an area, hospital, and sometimes it can be home hospice as well, where patients approach their end-of-life and often, or most of the time life support, especially coming from ICU is completely withdrawn and patients are entering hospice to basically approach the end-of-life that can sometimes range from hours, days to weeks and sometimes even months, depending on the situation... In today’s video, I want to talk about the clients that are at home already with a ventilator, with the tracheostomy but don’t have the right level of support, so case in point. Having been in the Intensive Care at Home space for over a decade now here in Australia, but also having been in the Intensive Care at Home space nearly 25 years ago in Germany, I have a fair bit of insight into this world. We are getting phone calls all the time where clients are at home on a ventilator with the tracheostomy, but they don’t have the right level of support i.e. they don’t have intensive care nurses 24 hours a day, which in many instances has simply been a death sentence, or clients go back to hospital all the time. So, once again, case in point. This week, I had a phone call from a prospective client who we’ve been engaging with for the last few months. His wife is at home on a ventilator with a tracheostomy for a rare neurological condition and he doesn’t have the support he needs. We are helping him currently with the advocacy process with the NDIS (National Disability Insurance Scheme). But the bottom line is, he ended up taking his wife to hospital this week because simply, the support workers that he’s got there cannot manage ventilator, tracheostomy, and in this instance, the oxygen saturation dropped. His wife had multiple secretions. He needed to suction her every five minutes, and that’s simply not the scope of a family member or a support worker, it’s the scope of an intensive care nurse. So, he ended up taking her to hospital because the NDIS, in many cases, lets people down and it’s trying to send support workers and then patients end up back in hospital all the time, that’s one of the better scenarios. But the reality is, we know of at least 7 patients in the last 10 years that have passed away at home because they didn’t have funding for 24-hour intensive care nurses, which by the way is also evidence-based. So, we’re not making this up because we’re running Intensive Care at Home, there’s actually evidence behind it. So a lot of the questions that we get when someone has a loved one in intensive care, on a ventilator with a breathing tube and they can’t wake up out of the induced coma and they can’t be weaned off the ventilator is whether a tracheostomy is inevitable or not. And they’re saying, every time that they try to wake up their loved one out of the induced coma, they’re breathing against the ventilator, they’re distressed, they’re agitated. And then the ICU team has to re-sedate them, put them back in a coma, therefore prolonging the induced coma and also prolonging the time on a ventilator. And the longer someone is on a ventilator, the less likely it is they can actually come off the ventilator, and then that’s when inevitably the discussions around a tracheostomy come up...
