We have also provided an emergency department bypass service for the Western Sydney Local Area Health District. We have successfully kept patients at home as opposed to them going to the emergency department. So, Intensive Care at Home can keep your emergency department free by providing some of that care at home as well. Now, in today’s video, I want to break down a case study but also talk more about that we are now doing Level 2 and Level 3 NDIS support coordination. The reason that is so important is that over the many years with Intensive Care at Home, we’ve been servicing clients now since 2013. We’ve provided hundreds of thousands of hours of Intensive Care at Home nursing at home. We have now also started to provide Level 2 and Level 3 NDIS support coordination because what we found over the years is simply that many clients that inquired for our service did not have the funding because the NDIS Support Coordinators didn’t do their jobs properly and they could only get funding for support workers, which in essence was a death sentence and patients have died because of it. So, that’s why we decided to provide our own NDIS support coordination. Recently, our NDIS Support Coordinator, Amanda, connected with a new client who’s at home with the tracheostomy, but only has support workers and that could be a death sentence. So, when support workers or even general registered nurses are looking after patients on a tracheostomy or on a ventilator, they don’t really know what to do. A tracheostomy is an artificial airway that in hospitals, most patients with a tracheostomy, 99% of patients in hospital with the tracheostomy would be in intensive care. That’s how unstable their airway is, and it takes specialist training for intensive care nurses to look after tracheostomy. So, for the NDIS to say, “We can look after tracheostomy at home with support workers and because they can do an online e-module” is serious business. It’s almost like a joke, but it’s not funny at all because people have died because of the incompetence from the NDIS because bureaucrats are making decisions without taking all the clinical assessments into consideration. It’s getting better now because we are, for example, doing all the nursing assessments for our clients and we are also engaging with the right NDIS Support Coordinators. So, I’ve now begun looking into several providers, but I’m still hit with roadblocks. Nobody seems to want to take my mother home with a tracheostomy needing dialysis. It’s currently up in the air as to whether someone can take her home or not. Is this something Intensive Care at Home can help with?” Well, thanks, Desiree, for sending your question through here. Now, this is something that we can absolutely help with. This is bread and butter for us. With going to dialysis as an outpatient, that’s certainly a possibility. Once your mom is at home with a tracheostomy, she needs 24-hour intensive care nurses at home because a tracheostomy is an artificial airway that requires the skill of intensive care nurses, 24 hours a day, to keep your mom out of hospital predictably. Few weeks ago, I’ve done a YouTube live with the title, “A blueprint for hospital discharge with tracheostomy with Intensive Care at Home.” I talked about an evidence-based discharge. So, what do I mean by that? Evidence-based means that home care for tracheostomy patients is evidence-based, and you can look up the evidence on our website intensivecareathome.com under the Mechanical Home Ventilation Guidelines. So, these guidelines are a result of over 25 years Intensive Care at Home nursing in Germany where it all started, and over 10 years of Intensive Care at Home nursing in Australia. Now, with Intensive Care at Home and clearly the Mechanical Home Ventilation Guidelines once again are evidence-based and say that only critical care nurses with a minimum of two years critical care nursing experience can keep patients at home safely on ventilation and/or tracheostomy. So, in order to get your mom home safely, I’m not surprised that you are hitting roadblocks because, there might be some providers out there who can say they can do that with support workers or with general registered nurses. In today’s blog post, I want to share a podcast with Amanda Riches, our Specialist NDIS (National Disability Insurance Scheme) Support Coordinator. Hi, Amanda. Amanda: Hi, Patrik. Patrik: Nice to see you again and thank you for coming onto the call. Amanda: No worries. Thank you very much for having me. Patrik: That’s great. So today, I really want to talk about Level 2 and Level 3 NDIS support coordination, and that’s why we brought Amanda on. Amanda is a very experienced Level 2 and Level 3 NDIS Support Coordinator. Obviously, it goes without saying that we are an NDIS approved or accredited service provider here in Australia. Amanda, tell us a little bit about your experience and why you joined Intensive Care at Home and some of the things we want to be working on with you. Amanda: Yeah, so I have been extremely fortunate to work in the disability sector for almost 30 years. I absolutely love what I do and really enjoy assisting people. So, I’ve got a lot of experience through from behaviors of concern all the way through to quite complex and spinal cord injury participants and assisting them to get the right outcomes. So, what brings me to Intensive Care at Home? So, Patrik and I started having a conversation and figured we were all pretty much on the same page. We had the same goals that aligned quite well and just want the best for the people that we support with the right training and education behind us as well to be able to support people properly. Patrik: I think, Amanda, one thing obviously, we are a nursing service first and foremost, and we have been dealing with NDIS Support Coordinators for many years. But one thing that led us to look for our own NDIS Support Coordinator, something that you and I discussed at length, which is simply that the quality of NDIS Support Coordinators across the board is not really suited to what our clients need who are very vulnerable, obviously with coming most of the time from ICU units on life support. Many NDIS support coordinators, with all due respect, they don’t really know what to advocate for these vulnerable clients. Obviously, you have worked in this space for much longer than I have when it comes to support coordination, but I think you have seen the pitfalls for clients and more importantly the gaps. You’ve identified the gaps, we’ve identified the gaps, which is why we think this is such a good match going forward. What have you identified in terms of gaps in this space? Amanda: I guess from a support coordination point of view, it’s not having the right supports in place for when people, like in our example, if people are leaving hospital and they need to liaise with the family and around supports that might be required, it’s the first NDIS plan for a lot of people... Now, let’s go into today’s quick tip. We’re working with a client at the moment who has their 57-year-old mom in ICU, after cardiac arrest, and potentially hypoxic brain injury. Now the client calls me this morning and says, “Hey, you need to know what’s happened. I just had a phone call from the ICU doctor saying they have started to test my mom for brain death because they’re thinking she might be brain dead.” So, this comes completely out of left field. The client had no idea that this was even a point of discussion and they now started with the brain stem reflex testing. And without showing the family the policy around brain death testing, without sitting down with the family saying, “Hey, look, we think your mom might be brain dead. These are the next steps”, no explanations whatsoever. And that’s really disappointing because I know ICUs can do much better... It’s quite clear that you can go home. You can just have a look at our case studies. We’ve been providing hundreds of thousands, if not millions of hours of Intensive Care at Home nursing now for similar clients in the community that were on the brink of dying in intensive care and they went home to improve their quality of life and in some instances, quality of end of life. Of course, I need to know more about your daughter’s situation. I need to know and understand all the ins and outs. However, let’s just say she is end of life, again, I don’t know the all the ins and outs. She can improve her quality of end of life at home and can have palliative care at home but that might not even be the case. Many of our clients have been in intensive care sometimes for years, in and out, or for years, months on end and intensive care units often wanted to withdraw treatment and then either the ICUs or the family found out about our service and then we have taken patients home and they have lived at home for long periods of times — years, and they want to live. They want to have a good quality of life and just taking someone out of the intensive care environment just makes a lot of sense. It’s a much more holistic and patient and family-friendly environment compared to an intensive care unit. This makes perfect sense. Intensive care units are depressing environments where you only want to be for short periods of times ideally for critical illness, and then you want to get out of there as quickly as possible. Now, there are the exceptions to the rule which, Eva, your daughter seems to fall in that category where she’s the exception. She’s been there for months on end, and she’s not going anywhere. So something that we’re observing and I’ve also observed it in over 20 years of working in intensive care is simply when patients are on a ventilator in an induced coma, and they have a nasogastric tube in the nose and they get nasogastric feeds, they need to be sitting up 30 degrees otherwise, the risk of aspiration is just too big. So what that basically means is, if patients are lying flat while they’re on a ventilator in an induced coma, and they’re having nasogastric tube feeds going in, and if they are lying flat, the risk of vomiting and aspirating gastric content into the lungs is very high and they would then often end up with aspiration pneumonia. So the number one goal for someone in intensive care in an induced coma, on a ventilator is to come off the ventilator and get out of the induced coma. If they’re aspirating stomach content because they’re lying flat and not sitting up 30 degrees at the minimum, that goal is jeopardized by simply the risk of aspiration pneumonia... So, today’s tip is about another success story that we had with one of our clients. So, I was actually talking to a client yesterday who was in ICU from September to December 2021, for over two months. At the time, this lady was 46 years of age. She was in ICU with lupus and COVID. She was on a ventilator with a breathing tube, was on high doses of inotropes. She was in kidney failure and things were looking pretty grim. Her sister, at the time, reached out to me asking for help and lo and behold, we went on a call with the ICU team who was painting this doom and gloom narrative, was painting this doom and gloom picture, and saying that her sister won’t improve and that she will not survive intensive care, and any treatment would be futile. Now, that was in the absence of an advanced care plan, i.e., her sister did not have documented what she would want in a situation like this...
